Functional Assessment Center
In May 2011, the Los Angeles County Department of Public Social Services contracted with St. John’s Well Child and Family Center to create a Functional Assessment Center that provides physical health disability assessments for residents of South Los Angeles. These assessments are critical in St. John’s service area, where 8.2% of residents are disabled and unable to work, a percentage nearly twice as high as in L.A. County overall. Assessment results determine patients’ disability status and whether they qualify for Supplemental Security Income, which entitles them to Medi-Cal coverage and a stable source of income. Additionally, patients seen at this clinic receive care coordination and benefits counseling to help them transition to receiving comprehensive services in a medical home. |
